1. A measure of socioeconomic position, e.g.

• educational attainment

• income

• deprivation

• occupational status

(Excluding studies that focussed on groups with one homogenous socioeconomic position)

2. Mesolevel access characteristics, e.g.:

• physician density at a regional level

• distance to physician

• travel time to physician

• office hours

(Excluding determinants of access at the macrolevel, e.g.: insurance status, provider payment schemes)

Outcomes

Any measure of individual healthcare use, e.g.

• physician visit

• hospital visit

(Excluding studies that focussed on health status or health-related behaviour, e.g.: physical activity, smoking)
